SLIP DISC, HERNIATED DISC, PROLAPSED DISC Intervertebral discs acts as shock absorbers between two vertebras. Discs have two parts, the inner portion and the outer portion. The inner portion is soft and gelatinous whilst the outer ring is tough. The inner portion of the disc can sometimes herniate to the outer portion due to weakness or injury. This is known as a slipped or herniated disc. There are times when the slipped disc compresses on one of the nerves emerging from the spine causing numbness and pain along the area of the affected nerve. Stages of Disc Prolapse • Disc Dehydration This is the stage where the outer layer of the disc weakens and the fibers around the disc become loose. This leads to flattening of the disc with loss of height and increase of the diameter. This is called a bulge. • Disc Prolapse This is a condition where the outer layer called the annulus fibroses cracks and the soft interior of the disc starts protruding through the outer layer. When this happens, the prolapsed part of the disc tends to touch on the adjoining nerves and it’s at this point that you start experiencing pain and other associated symptoms. • Disc Extrusion At this point, some part of the protruded nucleus pulposus starts to push out through the outer part of the disc and the gets detached. • Disc Sequestration In this stage, the free fragment of the nucleus pulposus (the inner most part of the disc) manages to get within the spinal canal. Who Gets Slipped Disc? Doctors have pinned age as a factor in many of the cases. In addition, smokers are also more likely to get slipped discs because smoking makes the discs lose their natural flexibility. Furthermore, overweight people are also more likely to suffer from a slipped disc because their disks have to support extra weight than they can handle. Ayurvedic Treatment for Intervertebral Disc Prolapse A prolapsed intervertebral disc is also known as slipped disc. A bulging out of the inner part of the disc is what happens in this condition. It’s a condition affecting the spine in which there is a tear in the outer fibrous ring of an intervertebral disc which allows the soft central portion to bulge. Symptoms: Backache. Numbness, parasthesia and weakness. Radiating pain as in Sciatica According to Ayurveda, the problem of an intervertebral disc prolapse occurs at three levels. Mamsa dhatu, asthi dhatu, and majja dhatu, are the three different tissue levels. Mamsa is the muscles; asthi represents bones, while majja denotes everything which is in the bones. Mamsa starts stiffness due to dryness, asthi becomes affected due to the muscles, and majja gets compressed due to the pressure effects. Due to the increased ruksha guna , the purpose of treatment is to reduce the dryness and restore the sneha guna, elasticity and also to rehydrate the disc. Sciatica is a general term for pain originating from the sciatic nerve. It is a severe painful condition where sciatic nerve of one or both the legs are involved. , it is a common disease which affects the movement of the legs. Often it is associated with low back pain. The condition where back pain and radiating pain of the legs are found together it is called Sciatica syndrome. In older times, as per Ayurveda system of medicine, it was named as Gridhrasi. One reason the sciatic nerve causes so much pain is because it is the longest nerve in the body! The nerve starts at the back of the pelvis and runs downward through the hip area and buttocks into each leg. Near the knee, the sciatic nerve divides into two nerves—the tibial and peroneal nerves. The tibial nerve runs behind the knee and the peroneal nerve runs along the side of the calf and ankle. Through the tibial and peroneal nerves, the sciatic nerve innervates (‘stimulates’) the action of many muscles in the lower legs and enables feeling in the thighs, legs, and feet. Etiological factors: All etiological factors of Vata imbalance disorders are the exclusive reasons of sciatica too. Common causes for sciatica- Intake of Vata aggravating eatables like- adhaki (pigeon pea), chanaka (Bengal gram), kalaya (peanut), masura (red lentil), nishpava (flat bean), uddalaka etc Excess intake of dry, light and cold food Excess intake of pungent, bitter and astringent food Heavy weight lifting, long walk, improper lie or sitting positions Suppression of urine, feces etc Improper practice of enema, purgation etc Fall or injury etc. Line of treatment: As it is one among Vata disorders, all vata pacifying measures should be undertaken. Snehana (oleation)- application, sprinking or pouring of oil over the affected area may be carried out. Swedana (sudation)- tub bath, pizichil, sudation with oil added medicated herbal pastes (patrapinda sweda), application of poultice (upanaha sweda) etc. are beneficial. Mridu samshodhana– mild purgation (Mridu Virechana) is preferred. Basti (medicated enema) Shiravyadha/ Rakta-mokshana (venesection or blood letting) – It is advised to perform 4 angula (3inch) above or below the knee joint.
